In event of a student absence please can a parent/carer complete the form below. If a parent/carer is unable to authorise this absence then the student may complete the form, but this must be indicated below and a parent/carer confirmation will be required as soon as possible.

 

Please note: If a Student declares they are filling out this form, all parent/carer e-mails are automatically checked against the students primary contact and an e-mail is sent to the parent/guardian informing them of the students absence.

Please select the date of absence.

Please select ALL DAY if you will not be in at all. Or If it is just one or a number of periods you are missing please choose them from the menu below. Please note you can select one or more periods.
